Program name: Community Health Representative, (LIHEAP) & Caregiver Support Program

Description: The Community Health Representative (CHR) is responsible for performing a variety of duties specific to home visits, client follow-ups and appropriate social service and medical activities. The Low Income Home Energy Assistance Program (LIHEAP) is to assist low income households in meeting their immediate home energy needs. The Caregiver Support Program provides assistance to anyone who is currently providing support services to an individual, who for reasons of Illness or frailty, cannot manage independently without assistance.

Eligibility & Requirements:

CHR:

For CHR transport you must have no other means of transportation. It must for medical reasons only unless specified. (CHR will be the one to determine what is allowable). CHR services all Native Americans at this time.

LIHEAP:

  • You must be of Seneca-Cayuga Tribe only, tribal card must be provided with application.
  • Copy of Current fuel bill showing; the date, applicant’s name or spouse’s name, current address, amount of bill, and vendor’s name.
  • Income verification for all adult members of household, 18 years and older. If unemployed, a letter from the Employment Office stating whether you do or do not receive benefits.
  • You must live within a 50 radius of Miami, OK.

Caregiver Support Program:

  • Applicants must be age 18 or over and meet the definition of a family caregiver. (“Anyone who is currently providing support services to an individual, who for reasons of Illness or frailty, cannot manage independently without assistance”)
  • Elders must be 52 or older and unable to perform at least two activities of daily living.
  • Applicant and elder must reside within the Seneca-Cayuga Tribe’s service area. (50 mile radius of Miami, OK)
  • Applicant and the elder or child must be Native American. (Copy of Tribal Card or CDIB required)
  • Ten percent of this program’s funding is for elders over the age of 52 raising children in their home who are under 18 years of age. These elders must be the primary care providers of the child(ren): however, legal guardianship is not required.
  • There are no income requirements at this time.

Priority of Service for caregiver support are as follows:

1.    Receiving Hospice

2.    Receiving home Health

3.    Others that need Caregiver Support Service.
