The Seneca-Cayuga Enrollment Committee will only accept the ORIGINAL Seneca-Cayuga Enrollment Applications distributed by the Seneca-Cayuga Tribe. Please do not download any Seneca-Cayuga Enrollment applications.
You may download the following form for change of address to notify the Tribal office. Please do not fax these forms. The original form MUST be mailed or taken to the Enrollment Office.
